A clear moderation report is what gets results certified — and what keeps you booked as a moderator. Here is a structure that quality-assurance bodies expect, written so an assessor can act on it.
Step 1 — Header and scope
State the provider, qualification, assessor, moderator, dates, and the sample moderated (how many PoEs out of the total, and why that sample).
Step 2 — Instruments and process
Confirm whether the assessment instruments were valid and aligned to outcomes, and whether the assessment process was fair, consistent and followed the plan.
Step 3 — Findings per criterion
For each sampled PoE, record whether the assessor's judgment was upheld, with evidence. Note sufficiency, authenticity, currency and relevance of evidence. Be specific — reference pages, not vague impressions.
Step 4 — Recommendations and sign-off
List required corrections, developmental feedback for the assessor, and your overall decision: results confirmed, or returned for action. Sign and date, and attach your moderation plan and sampling sheet.

Frequently Asked Questions
How big should the sample be?
Follow the provider's/ETQA's sampling policy — often a percentage across competent, not-yet-competent and borderline decisions to test consistency.
What if I disagree with the assessor?
Document it precisely and return those decisions for re-assessment with clear reasons. That is exactly the independence moderation exists to provide.
